This complex surgery involves harvesting not only a DIEP flap from the abdomen to rebuild the natural breast, but also lymph-nodes around the hip area which can be reconnected with their blood supply into the armpit after removal of axillary scar.

DIEP Flap with lymph node transfer is indicated in patients requiring natural tissue breast reconstruction after mastectomy who also suffer from upper arm and forearm lymphoedema.
DIEP flap with Lymph node transfer can be performed after mastectomy and radiotherapy were completed and lymphoedema to arm is already established or it can be performed to prevent lymphoedema occurrence. Dr Damir performs both techniques, depending on his patients’ individual circumstance.
Abdominal donor site scar is hidden under the underwear line in most patients whilst scar pattern in the reconstructed breast differs between immediate and delayed DIEP flaps. Lymphatic flap is hidden in the armpit.
Patients are discharged after 5-7 days and advised to avoid any strenuous physical activity or bending backwards/stretching abdomen for 6 weeks. Sleeping on the front or putting pressure on the flap are to be avoided and wounds should be kept dry for the first 2 weeks after surgery. After that very gentle scar massage with ointment is recommended daily.
